Childcare Forms:

Childcare & Medical Release Form – For small groups that meet off-campus yet utilize the childcare available at Christ Church, please download the childcare & medical release form and submit it to the nursery/childcare area. This form does not need to be completed for on-campus groups.

Childcare Reimbursement Form – If you are a Small Group Leader and you need reimbursement for your small group childcare, please download and complete the childcare reimbursement form and fax it back to Christ Church at 817-292-7630.
